Nationals entries deadlines

Nationals entries deadlines are listed below. To complete your nationals entry you must complete ALL of the following:

1) Fill out the TIVA on-line entry form. Instructions are listed on the website: http://www.tivaonline.com/tiva_login.asp

USAV membership numbers are available at:
http://www.yankee.org/usavnumb.html

2) Complete the seeding form, available on-line:
http://www.usavolleyball.org/media/adult/06Open/06seeding.pdf

3) Print out a copy of the official entry form from the TIVA site.

4) Write a check payable to "USA Volleyball Championships". Print team
name and division in memo section. Staple check to entry form.

$900 - club, masters, seniors
$475 - coed, reverse coed, u-volley, men's 80 and over, women's 70 and over

5) Mail entry form, seeding form and check to:

Entry deadlines:
March 6th: Entry deadline for 60's, 65's, 70's, 75's, 80's, U-Volley, Coed, Reverse Coed

March 27th: Entry deadline for B, BB, A, AA, 40's, 45's, 50's, 55's
